Publishing a Form

There are a variety of methods that a form is "made public".

1. Hosted on the department's letu.edu web page
2. Sent through an email
3. List on the University's Forms and Publications page

In order to retrieve the URL to publish:

1. Log in to letu.edu/formslogin and find the form you are ready to publish.  Click on Actions to the right of that form name:
   
2. In the options that pop up, click on Url Instructions:
   
3. Another window will pop-up.

If the form will be sent to non-LETU participants, use the URL for "Non-Single Sign On Forms".

If the form will be set to LETU participants, where they will be required to login to access the form, use the URL for "Single Sign On Forms".
