Uploading Form Submissions to eBridge

IT has built a process that uploads Dynamic Forms submissions to eBridge when requested (see "Add form to DF-to-eBridge process" Related Services to the right).  In order to prepare your form to upload to eBridge, perform the following steps:

  1. The Dynamic Forms Organization the form resides in should have a form that is named like one of the following:Uploaded Image (Thumbnail)
    These forms contain the structure and naming convention of each eBridge cabinet.
     
  2. In the form you are wanting to upload, add an Existing Item, select the Form Name that has the eBridge Cabinet table, then select the item that includes the name of the eBridge Cabinet.  Here is an example with Student Management:Uploaded Image (Thumbnail)
     
  3. Once it is added to the form, make sure to leave the field names as-is.  This is necessary for the process to properly upload and index the form submission in eBridge.  If a field needs to be removed or changed, the field names should be as follows:
    • Student Management Cabinet
      Dynamic Forms Field Name eBridge Index Name/Information
      eBridgeIndex1 {Student} Name (formatted Last, First Middle)
      eBridgeIndex2 {Student} ID
      eBridgeIndex3 SSN (this is typically excluded from forms due to its sensitve nature)
      eBridgeIndex4 Date of Birth
      eBridgeIndex5 Category
      eBridgeIndex6 Type of Document
      eBridgeIndex7 Miscellaneous
    • AP Cabinet
      Dynamic Forms Field Name eBridge Index Name/Information
      eBridgeIndex1 Vendor ID #
      eBridgeIndex2 Miscellaneous

  4. Since all form submissions will be the same or similar value, if you wish to change one of the short answer questions to a Choice List type question with limited options, just make sure that the field name exactly matches above.
  5. Once the form is complete and you are ready to start accepting submissions, make sure to submit a ticket using "Add form to DF-to-eBridge process" in Related Services to the right.
  6. The process runs every 15 minutes so you should begin to see any qualifying submissions in the eBridge cabinet shortly after confirmation that the form has been added to the process.
