Formstack is a cloud-hosted form and application service currently used for enrollment applications. When reporting an issue, describe, in as much detail, the exact nature of your issue and include names/IDs of the students and screenshots when applicable.

Possible issues can included (but are not limited to):
- Student applications not feeding into JX SEM
- Applications unavailable on the LETU website
