Faculty, staff and students utilize LimeSurvey in order to take polls, perform suverys, and otherwise gather information from specific populations. When reporting an issue, describe, in as much detail, the exact nature of your issue and include screenshots when applicable.

Possible issues can included (but are not limited to):
- Participant not able to access a survey after receiving an invitation email
- Not able to log into LimeSurvey after previously having access
