Cellular Service Approval - University Plan

The position approval for cellular service rests on the approval of the department VP and it is the department's ongoing responsibility to evaluate the needs for approved positions. The department's duty is to submit approved service requests,  service terminations, and device upgrades to IT. 

 

Cellular Replacement Cycle - University Plan

The is no replacement cycle, device replacement is implemented on an as-needed basis, and device upgrades or replacements are funded through the department with a supervisor's approval. To request a replacement device, use the link above for New Cell Service or Upgrade.

Note: unused or replaced devices are returned to IT to be data wiped. If the device has value, it is normally sold to a third-party vendor and proceeds are placed into the University Recovery Fund.

 

Stipend Request for Cellular Services - Personal Plan

This guideline is used to address the payment of communication fees incurred by LETU employees while conducting university business. This includes mobile services like cell phones, tablets, or internet hotspots (MiFi). 

  • Stipend eligibility is based on a position's duties; therefore, the supervisor should identify these positions and receive cabinet office approval (same as requesting a university plan).
  • Stipends will consist of a maximum payment of $55 per month.
  • Any changes to an employee’s status, stipend eligibility, or payments must be sent to HR.
  • Employees are responsible for establishing and maintaining their own mobile voice and/or data plans with their carrier of choice. They may add extra services or equipment (including equipment upgrades) as desired at their own cost and plan terms.

To request a stipend use the Dynamic Form under the HR Forms in SharePoint -> Payroll -> Faculty/Staff Action/Position Approval Form.  Please indicate in the ‘details’ that this is for a monthly mobile phone or Internet service stipend.
